Forms and Instructions for New Patients
The following are the forms all new patients must complete. Please either print and complete these forms, then email to info@gcot.net, fax to 817-884-7444, or bring them to your appointment. There is also an option to complete these forms electronically. If you prefer to complete electronically, please text 817-769-6937 and request the link for this option.
Please bring the following items with you for your first appointment:
- Current copies of all insurance cards
- Photo ID
- Names, addresses, and phone numbers of your current physicians
- Previous medical records relating to your visit, and all information related to your eye condition
- You will need to bring your glasses to your visit. If you wear contact lenses and are unable to bring your glasses, please bring a copy of your most recent glasses prescription from your eye doctor.
- Completed forms
- A form of payment for your co-payment, deductible, or any out-of-pocket amount required by your insurance.
IMPORTANT – PLEASE READ:
On your first visit, we make every effort to do a comprehensive exam and complete all testing, so please be prepared to spend two or three hours with us. Your eyes will be dilated, and there will be special diagnostic testing performed. We ask that you bring someone with you to drive you home since dilation of your eyes can last up to four hours.
